Advanced registration is required.  Please complete the registration form, enclose a check or money order, and mail to the address indicated on the form.  Registration will be accepted at the conference on a space available basis.

Contact Hour Disclaimers

The Arizona Perinatal/Neonatal Update conference will be requesting Nursing Continuing Education Contact Hours for this activity, based on the number of lectures scheduled.  It is anticipated that there will be a total of 6 contact hours per event day, for a possible total of 12 contact  hours.  Attendants may choose to attend one or both days.

In compliance with the contact hour accreditation provider (St. Joseph's Hospital and Medical Center) the following must be fully completed by each attendant in order to receive Contact Hours for each event day.

- Attendance of ENTIRE event day - all or nothing per day, no partial credits.  Attendants who arrive late or need to leave early will receive 0 credits for the day.

- Completion and submission of the ENTIRE evaluation form. Evaluations are both multiple choice and fill-in-the-blank.  Every question must be completed, as the accreditation provider requires a full report to verify attendants' participation.

- Arizona State Nursing License number must be provided.
