Advanced registration is required. Please complete the registration form, enclose a check or money order, and mail to the address indicated on the form. Registration will be accepted at the conference on a space available basis.

NNP/PAs, Nurses, Respiratory Therapists ........$ 300
Students ...........................................................$ 200
The fee includes a conference syllabus, handouts, continental breakfasts, lunches, coffee breaks and evening receptions. Cancellations will be accepted if received by October 14, 2012, however a $ 30 administrative fee will be deducted from the refund.
